Photocopy of Birth Certificate issued by the Municipality. |

Photocopy of Birth Certificate issued by the Hospital / Nursing Home. |
| 5(e) |
Address proof (any one of these : Aadhar Card / Voter ID Card / Passport / Electricity Bill / Driving License , of parents ). |

For Christians – Photocopy of Baptism certificate. |
| 5(g) |
Sibling proof – Photocopy of the latest mark sheet of Heritage Public School |
